Taking part in Millionaire for Life in Vermont is easy and can be a lot of fun. Here’s everything you need to get started:
- Choose Numbers: Grab a play slip from a Vermont retailer and select five numbers between 1 and 58 and Millionaire Ball between 1 and 5. Do it yourself or use Quick Pick for random numbers.
- Enter Multiple Draws: Up the excitement by entering multiple draws in a row. In Vermont, you can enter your ticket for 20 consecutive draws.

How to Claim Prizes
To claim prizes, sign the back of your ticket in the space provided to protect your rights. You will need to present the ticket to make your claim. How you claim prizes depends on the size of the prize:
- Less than $500: Prizes of this tier can be claimed at any authorized lottery retailer in the Green Mountain State.
- $500-$5,000: Must be claimed in-person at a Vermont M&T Bank or by mail.
- Jackpots: These must be claimed in-person at the Vermont Lottery Headquarters address below. You must provide your signed winning ticket, completed winner form, photographic ID such as your passport or driver’s license, and Social Security number.

Claim Period
You have 1 year from the date of the draw to claim your prizes. Be sure to provide all the relevant documentation to make your claim properly, depending on your prize tier.
Taxes
When it comes to taxes, prizes over $5,000 will typically incur a federal withholding fee as well as a Vermont state withholding:
- Federal Withholding: 24% for U.S. citizens and residents.
- Vermont State Withholding: 6% for Vermont residents.
Winnings of $600 and greater must be reported to the IRS and Vermont Department of Taxes. You will get a Form W-2G to include these amounts in your annual tax filing.
